41岁网红张雪峰跑步后突发心源性猝死的消息,前不久让无数人唏嘘惋惜。
近些年类似新闻频频出现:不少三四十岁、平时坚持长跑的中年人,甚至年轻健身爱好者,运动途中突发心脏意外。
一边是医生天天提倡运动护心,一边时不时爆出运动猝死案例,很多普通人陷入困惑:运动到底养身还是伤身?怎样运动才能避开血栓、心梗风险?
发表在《欧洲临床营养学杂志》的一项多国联合试验,从凝血规律、咖啡、运动时段三个维度,给出了很实用的科学答案[1]。

只要一动,血液就会短暂变“黏稠”,是人体自带的生理机制
研究选取30名久坐、几乎不喝咖啡的年轻男性,分时段做标准化中等强度有氧运动,抽血监测凝血指标后发现:运动之后,人体血小板聚集、凝血酶生成都会上升,血块溶解时间变长,血液短期更容易凝固。

这不是身体的bug,而是人体的自我保护机制:运动过程中肌肉、微血管难免出现微小破损,身体提前上调凝血能力,能防止运动受伤后大出血。
但血液容易凝固,意味着更容易产生血栓。血栓事件是心血管疾病(CVD)的主要诱因之一。
好在人体自带平衡系统,运动后体内溶栓物质tPA同步升高,主动溶解多余血栓。
对身体健康、无基础病的普通人来说,运动导致的“血液粘稠”会自行恢复,基本不会形成血栓。但如果叠加一些其他的刺激性因素,身体的平衡系统就很容易被打破。

傍晚运动更安全
虽然不管是早上还是傍晚的运动,都会增加血栓的风险,但是早晨的风险更高。
早晨运动后,监测到更高的血小板聚集、更强的凝血酶生成、更高的PAI-1活性以及更长的血块溶解时间,即纤溶能力更差,更容易形成血栓。
相反,傍晚运动后,则监测到更低的促血栓指标和更强的纤溶能力(tPA活性更高,血块溶解时间更短),即溶解血栓的能力更强。

之所以有这个差别,是因为有人体昼夜节律的影响:
PAI-1的晨峰现象:早晨人体内的PAI-1水平自然达到峰值,这抑制了纤溶系统,导致血凝块更难被溶解。
血小板反应性:早晨起床后,血小板反应性增强,增加了聚集倾向。
交感神经活动:早晨交感神经张力较高,可能进一步放大运动后的凝血反应。
事实上,早晨是心血管事件的高发时段,这与早晨身体天然的“促血栓状态”有关。
而这个研究说明,运动可以在早晨进一步强化“促血栓状态”。
需要指出的是,如果作息时间不是早睡早起,那么“早晨”也可以延伸到中午的时段。
根据媒体报道,张雪峰在公司跑步后出现不适的时间,是中午12点26分。

早起空腹喝咖啡立刻运动,会放大血栓风险
很多上班族习惯晨起一杯美式提神,紧接着出门跑步、健身,这貌似非常健康的生活方式,恰恰是血栓风险最高的搭配。
在上述的试验中也测试了咖啡因对血栓的影响。相比不喝咖啡的人,受试者喝完咖啡后晨练,会出现血小板抱团更明显、凝血酶更多、血块更难溶解的现象。
简单来说:运动本就让血液易凝结,晨起咖啡因相当于火上浇油,进一步拉高血栓潜在风险。
在上述试验中,参试者咖啡因摄入量为每公斤体重3mg,相对于1杯普通美式咖啡的量。
当然,由于不同的人对咖啡因的耐受不一样,咖啡在不同的人中,效果也会有差距。
研究也测试了同等剂量咖啡在傍晚的效果,发现促凝作用大幅减弱,即便喝咖啡再运动,凝血指标波动也温和很多。
所以,傍晚运动更安全,傍晚喝咖啡对血栓的影响也更小。(但是对睡眠的影响就会更大……)

优选傍晚锻炼,避开晨练配咖啡的误区
需要指出的是,上述的研究有一定的局限:试验受试者全是平日久坐、几乎不喝咖啡的年轻男性。
久坐本身就是导致心血管疾病的坏习惯。长期不动容易让血小板异常活跃、身体溶栓能力变差,血液偏向易凝状态。最典型的例子:长途坐飞机久坐诱发下肢血栓。
因此,改变久坐的行为,可以减少心血管风险。在长途飞行中,也更要注意多走动,或者做一些抬腿运动。
上述的研究特意挑选久坐人群做试验验,是因为这类人指标波动更突出,咖啡因和运动带来的凝血变化更容易被观测出来。
所以这个试验的目的绝非劝大家放弃运动,而是帮我们找准喝咖啡和锻炼的合理时机。
结合实验数据,日常健身记住三点就能大幅降低心血管隐患:
锻炼尽量选傍晚:傍晚六点前后身体溶栓能力最优,溶栓物质 tPA 分泌旺盛、促血栓因子更少,同等运动量下血液不易变“稠”。
千万别晨起空腹喝咖啡立刻运动:三高、有血栓史、心脑血管家族史以及 60 岁以上人群,千万别踩这个组合。早起实在想喝咖啡,建议练完休息一小时再饮用;晨练前只喝温水,弥补夜间缺水。
久坐人群循序渐进健身:长期不动不要骤然长跑猛练,先从散步等低强度运动开始;运动过后若是持续胸闷、小腿酸胀、莫名乏力,马上停练及时就诊。

结语
看别人彩票中奖,觉得自己也行,是草率;看别人运动猝死,觉得自己也不行,更是草率。
只盯着极端个例,不学整体概率,这种 “片面认知”,远比不懂更害人。
科研的意义,就是帮我们看清概率、区分偶然和常态。
不必被个别猝死新闻吓退,规律运动护血管、稳三高的收益,远远大于极低风险。
真正伤身的,从来不是运动本身,而是错的时间、错的习惯,撞上了认真的运动。

On one hand, doctors advocate for exercise to protect the heart every day, while on the other hand, there are occasional cases of sudden death from exercise. Many ordinary people are confused: is exercise beneficial or harmful? How to exercise to avoid the risk of blood clots and heart attacks?
A multinational joint trial published in the European Journal of Clinical Nutrition provides practical scientific answers from three dimensions: coagulation patterns, coffee consumption, and exercise duration.

A study selected 30 young men who sit for long periods of time and hardly drink coffee, and did standardized moderate intensity aerobic exercise in different time periods. After monitoring coagulation indicators through blood sampling, it was found that after exercise, platelet aggregation and thrombin production in the human body would increase, blood clot dissolution time would be longer, and blood clotting would be easier in the short term.
This is not a bug in the body, but a self-protection mechanism of the human body: during exercise, muscles and microvessels are inevitably slightly damaged, and the body upregulates its coagulation ability in advance, which can prevent heavy bleeding after sports injuries.
But blood is prone to clotting, which means it is more likely to produce blood clots. Thrombotic events are one of the main causes of cardiovascular disease (CVD).
Fortunately, the human body has its own balance system, and after exercise, the thrombolytic substance tPA in the body increases synchronously, actively dissolving excess blood clots.
For ordinary people who are physically healthy and have no underlying diseases, the "blood viscosity" caused by exercise will recover on its own and will not form blood clots. But if combined with some other stimulating factors, the balance system of the body is easily disrupted.

Exercise in the evening is safer
Although both morning and evening exercise increase the risk of blood clots, the risk is higher in the morning.
After morning exercise, higher platelet aggregation, stronger thrombin production, higher PAI-1 activity, and longer clot lysis time were detected, indicating poorer fibrinolysis ability and increased likelihood of thrombus formation.
On the contrary, after evening exercise, lower thrombotic indicators and stronger fibrinolysis ability (higher tPA activity and shorter clot lysis time) were detected, indicating a stronger ability to dissolve blood clots.
The reason for this difference is due to the influence of the human circadian rhythm:
The morning peak phenomenon of PAI-1: The level of PAI-1 in the human body naturally reaches its peak in the morning, which inhibits the fibrinolytic system and makes blood clots more difficult to dissolve.
Platelet reactivity: After waking up in the morning, platelet reactivity increases and the tendency to aggregate increases.
Sympathetic nervous activity: In the morning, sympathetic nervous tension is higher, which may further amplify the coagulation response after exercise.
In fact, the morning is a high-risk period for cardiovascular events, which is related to the body's natural "thrombotic state" in the morning.
And this study suggests that exercise can further enhance the "thrombotic state" in the morning.
It should be pointed out that if the sleep schedule is not early to bed and early to rise, then "morning" can also be extended to the noon period.

Drinking coffee on an empty stomach in the morning and exercising immediately can increase the risk of blood clots
Many office workers are accustomed to having a refreshing American drink in the morning, followed by running and exercising outside. This seemingly healthy lifestyle is precisely the combination with the highest risk of blood clots.
The effect of caffeine on thrombosis was also tested in the aforementioned experiment. Compared to those who do not drink coffee, participants who exercise in the morning after drinking coffee tend to have more obvious platelet clumping, more thrombin, and more difficult blood clots to dissolve.
Simply put, exercise makes blood easier to clot, and caffeine in the morning is like adding fuel to the fire, further increasing the potential risk of blood clots.
In the above experiment, the participants had a caffeine intake of 3mg per kilogram of body weight, relative to the amount of one cup of regular American coffee.
Of course, due to different people's tolerance to caffeine, the effectiveness of coffee may vary among different individuals.
The study also tested the effect of the same dose of coffee in the evening and found that the procoagulant effect was significantly reduced. Even after drinking coffee and exercising, the fluctuations in coagulation indicators were much milder.
So, exercising in the evening is safer, and drinking coffee in the evening has less impact on blood clots. (But the impact on sleep will be even greater...)

Choose to exercise in the evening and avoid the misconception of pairing morning exercise with coffee
It should be pointed out that the above study has certain limitations: the participants in the experiment were all young men who sit for long periods of time and hardly drink coffee.
Sedentary sitting itself is a bad habit that leads to cardiovascular disease. Long term inactivity can easily lead to abnormal platelet activity, decreased thrombolytic ability, and a tendency for blood to become more prone to coagulation. The most typical example: Long distance flight and prolonged sitting can induce lower limb thrombosis.
Therefore, changing sedentary behavior can reduce cardiovascular risk. During long-distance flights, it is also important to pay attention to walking more or doing some leg lifting exercises.
The above study specifically selected sedentary individuals for experimental testing because their indicators fluctuate more prominently, and the coagulation changes caused by caffeine and exercise are easier to observe.
So the purpose of this experiment is not to persuade everyone to give up exercise, but to help us find the right time to drink coffee and exercise.
Combining experimental data, remembering three points in daily fitness can significantly reduce cardiovascular risks:
Exercise in the evening as much as possible: Around 6pm in the evening, the body has the best thrombolytic ability, with strong secretion of the thrombolytic substance tPA and less thrombogenic factors. Under the same amount of exercise, the blood is less likely to become "thick".
Never drink coffee on an empty stomach in the morning and exercise immediately: for those with high blood pressure, history of blood clots, family history of cardiovascular and cerebrovascular diseases, and those over 60 years old, do not step on this combination. If you really want to drink coffee early in the morning, it is recommended to rest for an hour after practicing before drinking; Drink only warm water before morning exercise to make up for the lack of water at night.
Gradual fitness for sedentary individuals: avoid sudden long-distance running and intense exercise when not moving for a long time, start with low-intensity exercises such as walking; If you experience persistent chest tightness, leg soreness, or unexplained fatigue after exercise, stop exercising immediately and seek medical attention promptly.

Conclusion
Watching others win lottery tickets and thinking that one is also capable is hasty; Watching others die suddenly during exercise and feeling that I am not capable is even more hasty.
Focusing only on extreme cases without learning the overall probability is far more harmful than not understanding.
The significance of scientific research is to help us see probabilities and distinguish between chance and normalcy.
Don't be intimidated by individual sudden death news, the benefits of regular exercise for protecting blood vessels and stabilizing the three highs far outweigh the extremely low risks.
The real harm to the body is never the exercise itself, but the wrong timing and habits that collide with serious exercise.
